Drug Utilization Evaluation of Antibiotics in Orthopedics
Surgery in a Hospital in India

Background: Antibiotic resistance, a major consequence
of antibiotic overuse, is an important problem worldwide. The Intensive
care unit (ICU) is a setting where a large number of drugs are administered
to patients & chances for drug errors are high.
Objectives: To assess the pattern of antibiotics usage in the Intensive
care unit (ICU) & High dependency unit (HDU) in a hospital in
New Delhi (India).
Methods: A retrospective analysis (qualitatively and quantitatively)
of the patients admitted to the ICU & HDU was conducted for a
duration of four months. The present study was carried out to obtain
basic demographic information of patients i.e co-morbid, H/O allergy,
past surgery, drug utilization during the study period, outcomes,
duration of hospitalization, time taken in surgery, culture sensitivity
reports, age, sex, weight distribution of patients were also studied,
empirical or definitive therapy and to measure antibiotics consumption
monthly.
Results: Out of 111 patients who have been admitted during the study
period, 60(54.05%) male & 51(45.95%) female were audited. 50.45%
of patients were above 55 years & 72.07% of patients were above
60 kg. The average mean duration of hospitalization was 6.29±4
days. E.coli, P.aeruginosa were the common organism isolated from
the infected patients. The average exposure of patients to antibiotics
for prophylaxis was 1.79±0.43, for treatment was 2.04±0.86,
on discharge 1.18±0.39. 88.29% of patients were given empirical;
11.71% were given definitive therapy. Frequently prescribed antibiotics
or their combination were cefuroxime, cefuroxime+ tobramycin. Consumption
of cefuroxime was maximum followed with tobramycin.
Conclusion: The use of antibiotics on empirical basis was a routine
prescribing practice. Second generation followed by third generation
cephalosporins are the most popular antibiotics in combination with
aminoglycosides. The study finding call for a periodic review of drug
utilization evaluation of antimicrobials in the ICU & H DU.
