F. Zerehsaz
Iran

Objective: -Here, we report 2 patients with drug resistant scrofuloderma (Tuberculous-adenitis) and one patient with resistant lupus vulgaris that were successfully treated with a herbal mixture named “Z-HE”

Material and Method: Patient no. 1 : A 21 years old boy was referred from Shiraz T.B. center having scrofuloderma of 2 years duration, was on anti-tuberculous therapy( INH, rifampin, ethambutol, pyrazinamid and) after which he developed INH hepatitis and got jaundice. Then, he underwent surgery (right & left functional neck dissection with removal of all cervical lymph nodes). There was no cure with draining fistula.
The patient proposed for our topical “Z-HE” treatment for two weeks. Cure was evident, there was no more discharge and the fistula healed. Also, there was no relapse in 12 years of follow-up.

Patient no. 2: An 18 years old girl was also referred from Shiraz T.B. center having scrofuloderma of 9 months duration. Anti-tuberculous therapies were non-conclusive, Proposed for the topical “Z-HE” treatment for 15 consecutive days. Cure was evident with no relapse in 2 years of follow-up.
Patient no. 3: A 5 years old girl having lupus vulgaris of 2 years duration. Conventional therapy was non-conclusive, proposed for the topical “Z-HE” treatment for 15 consecutive days. There was complete cure with no relapse in 2 years of follow-up.

“Z-HE” is a mixture of herbs used as a topical paste in treating cutaneous leishmaniasis successfully for the last few years.

Conclusion: This is probably the first time treating successfully resistant extra-pulmonary tuberculosis with a topical herbal remedy, that can be a quite promising drug. Our proposal for this mixture of herbs may be through differentiating Th1T-cell lymphocytes and or blocking Th2T-cell differentiation, which need further work-ups & investigations.
