Advanced Penile Verrucous Carcinoma
Treated With Intra-Aortic Infusion Chemotherapy.

Purpose: Traditionally, the treatment for advanced penile
verrucous carcinoma is penectomy. This mostly leads to remarkable
psychosexual problems and greatly affects the quality of life especially
in young patients. In order to preserve the penis, intra-aortic
infusion chemotherapy was used in advanced verrucous carcinoma .
Materials and Methods. From
1991 to 2009 6 cases of penile verrucous carcinoma were treated
by continuous intra-aortic infusion with methotrexate (50mg) every
24hrs, for an average of 11 days. Citrovorum factor (6mg) was given
intramuscularly every 6 hours during the period of methotrexate
infusion.
Results: After treatment, four patients achieved complete
remission. They are all living disease free from17years10months,
to 3 years9months after therapy. Partial responders subsequently
received penectomy. One case developed transformation to squamous
cell carcinoma with inguinal metastases and died.
Conclusions: Intra-aortic infusion chemotherapy is a simple
and effective method for penile verrucous carcinoma with the uniqueness
of preservation of anatomical structure, sexual function and fertility
for the complete responders. For penile verrucous carcinoma especially
in younger patients, intra-aortic infusion chemotherapy may be considered
as an organ-sparing treatment before penectomy is carried out.
